For K-8 Student Records, please email the completed
Student Records Request Release Form and a copy of your government-issued ID to sarah.torres@prescottschools.com

For 9-12 Student Records, please visit the Parchment website or contact Sheri Landino, PHS Registrar at sheri.landino@prescottschools.com

Special Education Records must be requested from Exceptional Student Services: kathleen.knutson@prescottschools.com

Requests may also be mailed or hand-delivered to:
