Insurance
We encourage you to maintain an insurance policy of your choice for your child(ren).
Prescott Unified School District does not provide accident/injury insurance on your child. However, each year you will have the option of obtaining a low cost accident/injury insurance through an independent insurance company, offered through the school district. Information about this insurance is sent home at the beginning of each year by school personnel. This form must be completed and returned to the school office if you wish to provide this coverage for your child.
Please remember that if you choose to participate in the insurance program, the claim transaction is between you and the insurance company. If your child is injured, a claim form may be obtained from the school office.
