For your convenience, the PUSD elementary enrollment form is available to print. The enrollment form is part of an enrollment packet that includes a home language questionnaire, a special needs questionnaire, an immunization record sheet, a student health survey, an entry survey and immunization requirements information -- all of which are distributed by the school on the day of enrollment.

Elementary School Enrollment Information

School starts Monday, August 9, 2010.

2010-2011 registration resumes July 19th at the individual elementary schools.

Elementary schools are closed for the summer and will re-open for enrollment on July 19, 2010. To enroll your child in a PUSD school, please go to your neighborhood school or school of choice when it re-opens. Parents are encouraged to register as soon as possible to ensure placement in the school of choice. Please bring your child's birth certificate and immunization records. Arizona law requires that parents provide verification that immunizations have been completed prior to starting school.

Children who turn five years of age before September 1, 2010, are eligible for kindergarten in the 2010-2011 school year.

OPEN ENROLLMENT
Arizona’s Open Enrollment law mandates that all Arizona school districts allow transfers between schools and school districts based on space availability. With this is mind, parents are encouraged to register as soon as possible. Because of the open enrollment law, students are granted enrollment in a school based on the following priorities (enrollment is always based on space available):

-New students living in attendance area granted enrollment immediately
-New in-district (but out-of-attendance-area) students granted placement March 8
-New out-of-district students registering March 1 - May 31 granted school placement June 1
