Partners for Healthy Students


~ A school based health program sponsored and promoted by PUSD in partnership with Yavapai Regional Medical Center ~

Partners for Healthy Students is a free school-based clinic established by Yavapai Regional Medical Center that brings health services to the school for students with no health insurance or who are underinsured. A Pediatric Nurse Practitioner delivers care and services which includes:

bullet Diagnosis and treatment of illnesses
bullet Prevention/well child visits
bullet Sports physicals
bullet Prescription medicine, when required

Uninsured - No AHCCCS, no KidsCare, no insurance
Underinsured - Deductibles higher than $1000 and no co-pays when you visit your health care provider

The mission of Partners for Healthy Students is to provide school based health care services to uninsured and underinsured students.

bullet To provide access to basic health care services for uninsured and underinsured students.
bullet To access other available health and community resources.
bullet To decrease school absenteeism due to illness.
bullet To collect data in order to document outcomes and design needed programs.

Tuesday - Lake Valley Elementary / Humboldt Unified - 9am - 4pm
Wednesday - Territorial Elementary / Chino Valley Unified - 830am - 3pm
Thursday - Bradshaw Mt High School East / Humboldt Unified - 830am - 12noon
Thursday - Dexter Family Resource Center / Prescott Unified 130pm - 4pm
