Each sport requires a new parent permission form, school insurance/private health insurance, payment and physical form (for football and wrestling). Insurance is required for every sport. If you do not have health insurance for your child, you may purchase school insurance from Myers-Stevens & Toohey & Co., Inc. Insurance forms are sent home with students.
You will receive a receipt for any money paid for sports payment for tax credit purposes. This may be applied toward your $400 per family tax credit amount per year.
Eligibility
Extracurricular activities are all interscholastic activities in which the students represent the school in competition, or by participating in activities such as band performances, student council, choir, or other activities for which no credit is earned. These extracurricular activities operate under the guidelines established by the Arizona Interscholastic Association. Mile High has adopted a three-week eligibility period. Any student receiving an F on a three-week progress report will be considered ineligible for the following three-week period. The student may return to eligibility by receiving no failing grades on the next progress report. No athlete may participate in any sport if they are 15 years old before September 1st of the current year. In all cases, Mile High coaches and administration will have the final say in matters of student athlete eligibility.
