Voting Item: 1. Consider approving the 2003-04 Annual Financial Report – Mrs. Renee Raskin, Director of Business Services. (Please refer to Board Briefing #283, Item No. 1.)

Consider approving the hiring of four 6-hour per day instructional assistants to work in 1st grade classrooms for the remainder of the 2004-205 school year only, utilizing 2004-2005 student growth monies – Dr. Chris Reynolds, Assistant Superintendent (Please refer to Board Briefing #283, Item No. 2.)

3. Consider approving an out-of-state travel request for the Prescott High School Boys’ Soccer Team to travel to Lake Charles, Louisiana on December 15-19, 2004, for the Louisiana Showcase of Soccer – Mr. Kevin J. Kapp, Superintendent. (Please refer to Board Briefing #283, Item No. 3.)

4. Consider approving the Governing Board Goals for the 2004-2005 school year – Mr. Kevin J. Kapp. (Please refer to Board Briefing #283, Item No. 4.)

5. Consider approving a resolution to the Arizona School Boards Association that would support the ASBA By-Laws being amended to allow the two exofficio members (the President of the Hispanic Native American Indian Caucus and either an officer or director of the National School Boards Association) to vote during any ASBA Board of Directors meeting – Mr. Kevin J. 6. Consider approving the acceptance of a grant from the Acker Trust Board for fiscal year 2005 for $3,000 for the Prescott High School Music Program – Mr. Kevin J. Kapp, Superintendent. (Please refer to Board Briefing #283, Item No. 6.)

7. Consider approving a new program at Prescott High School in partnership with Vocational Rehabilitation entitled "Youth Transition Program" to better serve transition needs of Special Education students from school to work and adult life – Mr. Kevin J. Consider approving any nominations of school board members for the ASBA Honor Roll Award for recognition of outstanding school board members with 8 or more years of service through the Arizona School Boards Association – Mr. Kevin J. INFORMATION ITEMS

18. Mrs. Raskin has prepared the School Board Summary Budget Report for your review. (Please refer to Enclosure #14.)

19. Attached for your review is a commendation letter regarding Mr. Jay Collier and his assistance in the construction of the permanent concession stand located at the PHS football field from Ms. Tani Odorfer of the Badger Football Boosters. (Please refer to Enclosure #15.)

20. The following are continuation grants which have been awarded to Prescott Unified School District submitted by Mary Anne Kapp:

School Safety Program

2003-2004 Allocation: $62,063.00; 2004-2005 Allocation: $62,073.00

The School Safety Program provides salary and travel for a School Resource Probation Officer for Prescott High School, Granite Mountain Middle School and Prescott Mile High Middle School. This program assists in teaching Law Related Education for a safe school environment. (Please refer to Enclosure #16.)

Title VII Indian Education submitted by Mary Anne Kapp

2003-2004 Allocation: $24,072.00; 2004-2005 Allocation: $25,041.00

The Title VII Indian Education Formula grant develops and implements elementary and secondary school projects that will increase educational opportunities for and meet the unique educational and culturally related academic needs of Indian children. (Please refer to Enclosure #17.)

DES (Department of Economic Security)

2003-2004 Allocation: July 1, 2003 to June 30, 2004 ~ $7,380.00

2004-2005 Allocation: July 1, 2004 to December 31 ~ $3,678.00

Funding is for school-based prevention programs to address academic failure, substance abuse or behavioral concerns. Children and their families will be provided with the necessary skills to promote a successful school experience. (Please refer to Enclosure #18.)

Intergovernmental Agreement for the North Central Arizona Mathematics and Science Consortium

The Arizona Department of Education has awarded the North Central Arizona Mathematics and Science Consortium a contract for assistance in providing workshops for educators throughout Yavapai County. Prescott Unified School District has joined the Consortium based upon 2003-2004 ADM for $3,764.25. (Please refer to Enclosure #19.)
